Testing a Free-T4, a Free-T3, a TSH and a Thyroid peroxidase antibody allows us to measure the state of the thyroid.
If the pituitary gland detects a drop in the level of thyroid hormones in the blood it increases the production of Thyroid Stimulating Hormone (TSH). In a normal person, this causes the thyroid gland to produce more T4 which is converted to the active hormone; T3.
If the thyroid gland is under active (Hypothyroidism) this increase in TSH will have little so the Free T4 and Free T3 will remain low and the TSH will climb higher. Conversely, if the thyroid gland is overactive (Hyperthyroidism) then the Pituitary will stop producing TSH and the Free-T4 and Free-T3 will be high. One of the common reasons the thyroid gland fails is due to the body attacking it with antibodies, the most common antibody is anti-Thyroidperoxidase.
